F5 form for individual investors from Saskatchewan
The F5 risk acknowledgement form is for individual investors from Saskatchewan who are completing the Friends and Family exemptions. Only “close personal friends” or “close business associates” must complete this form—investors who are using the family member category do not need to complete this form.
